** Advisor's Club Book Order Form
Up one levelYour order may be either mailed, emailed, or dropped off at the Extension Office. Remember when you print, it is on legal size paper. Faxing this form WILL NOT WORK since our fax machine does not accept legal size paper. ALL book orders MUST be on this form. Due to the time needed to attach the insert pages to each project book, PLEASE allow 2 workign days for us to have your books ready for pick up.

- This form is 2 pages, on legal size paper. It is in Microsoft Word. You can save a copy of the file to your computer, enter your order and the email your new file back Jill in the Extension Office (ellis.301@cfaes.osu.edu). If you turn off the "insert" button, everything will stay in place as you type in your information!
